Role Description The Care Navigator is a flexible community based role supporting patients in the Dayton Oh area. In this position, the Care Navigator works directly with patients to understand their needs, explain available services, and guide them through the healthcare and social services systems. Daily activities include conducting outreach and assessments, scheduling and coordinating appointments, and following up to ensure care plans are understood and completed. The role also involves identifying community resources, making referrals to support services, and tracking outcomes in documentation systems. Care Navigators collaborate with clinical teams, community organizations, and families to promote continuity of care and address barriers to wellbeing.

Qualifications

  • Experience in patient or client-facing roles such as care coordination, community health work, or social services.
  • Strong communication, active listening, and interpersonal skills, with the ability to build trust across diverse populations.
  • Organizational and time management skills, including maintaining accurate records and managing multiple cases or tasks.
  • Knowledge of local community resources, healthcare systems, and Social Determinants of Health, or willingness to learn quickly.
  • Ability to work in the community in and around Dayton, OH, collaborate within interdisciplinary teams, and adapt to changing needs and priorities.
  • Basic proficiency with computers and data entry in electronic systems; familiarity with care management software is a plus.
  • High school diploma or equivalent required
  • Experienced in community based care- CHW, LPN, MA, EMT basic, MA, Previous Medicaid Case Worker
  • Prior experience in healthcare, nonprofit, or community-based organizations is beneficial.
