Opportunity Type: Full-Time (30 Hours Per Week)
Office Location: Hingham, MA
Service Area: South Shore, including Fairhaven, New Bedford, Somerset, and Centerville (up to 53 miles from Hingham).
Pay Range$38.20- $39.03/Hour*
*Final offers are based on experience, qualifications, skill set, and location.
Sign-On Bonus: $2,000 Full-Time (30+ hours)
Bonus paid in two increments; terms apply.

About the Role
The Adult Family Care Registered Nurse (RN) plays a critical role in supporting adults with disabilities and complex health needs who live in community-based family care settings. As part of our multidisciplinary Adult Family Care team, the RN provides clinical oversight, health monitoring, care planning, caregiver training, and advocacy to help ensure the health, safety, and well-being of AFC members.
This position combines clinical expertise with relationship-building, allowing you to work closely with members, caregivers, healthcare providers, and community partners to promote person-centered care and enhance quality of life.
What You’ll Do
- Conduct nursing assessments for prospective and enrolled AFC members.
- Develop, implement, monitor, and update individualized care plans.
- Complete required health status reports, reassessments, and clinical documentation.
- Conduct onsite visits with AFC members and caregivers every other month, or more frequently as needed.
- Monitor member health, identify changes in condition, and recommend interventions and supports.
- Train and support caregivers on healthcare needs, medication management, infection prevention, and physician-ordered interventions.
- Collaborate with Care Managers, physicians, specialists, hospitals, pharmacies, and community providers.
- Review physician orders, caregiver records, and health-related documentation.
- Participate in admissions, orientation, discharge planning, and transitions of care.
- Respond to urgent health and safety concerns and support crisis intervention efforts.
- Help ensure compliance with MassHealth AFC regulations and organizational standards.
- Participate in quality improvement activities, audits, and program development initiatives.
What We’re Looking For
Required Qualifications
- Current unrestricted Massachusetts Registered Nurse (RN) license.
- Minimum of two (2) years of recent experience providing direct care to elders and/or adults with disabilities.
- Valid driver’s license, reliable transportation, current automobile insurance, and acceptable driving record.
- Strong assessment, documentation, organizational, and critical thinking skills.
- Excellent written and verbal communication skills.
- Ability to work independently, manage a caseload, and collaborate effectively within a team environment.
- Proficiency with Microsoft Office and electronic health record systems.
- Commitment to person-centered services, inclusion, dignity, and individual choice.
Preferred Qualifications
- Bachelor’s degree in nursing (BSN).
- Experience working with community-based healthcare, long-term services and supports, or disability services.
- Knowledge of medical, behavioral health, social service, and community support systems serving adults with disabilities.
