About the Opportunity
A respected nonprofit organization serving individuals with intellectual and developmental disabilities is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to support residential and community-based programs. This role offers the opportunity to make a meaningful impact by helping individuals maintain their health, wellness, and quality of life while working alongside a collaborative interdisciplinary team.
This position provides a stable weekday schedule with flexibility to accommodate qualified candidates and includes limited local travel between nearby program locations. Part time and full time openings!
Key Responsibilities
- Monitor and support the healthcare needs of assigned individuals.
- Conduct nursing assessments and assist with ongoing health monitoring.
- Administer medications and oversee medication-related processes.
- Collaborate with caregivers, program staff, healthcare providers, and family members.
- Provide medical guidance and training to direct support staff.
- Assist with care planning, risk management, and health-related documentation.
- Ensure compliance with applicable healthcare regulations, policies, and procedures.
- Support individuals following hospitalizations, medical appointments, or changes in health status.
- Participate in team meetings and contribute to person-centered care planning.
- Maintain accurate and timely electronic health records.
Qualifications
- Active Licensed Practical Nurse (LPN) license in the State of Indiana.
- Minimum of 2 years of nursing experience preferred.
- Experience supporting individuals with developmental disabilities, behavioral health needs, long-term care, residential services, or community-based programs is a plus.
- Strong communication and interpersonal skills.
- Comfortable using electronic documentation systems and basic computer applications.
- Valid driver’s license and reliable transportation.

Pay: $28.00 – $31.00 per hour
