Responsibilities: Coordinate the Pediatric and Adult Diabetes Program by providing self-management education and resources to patients and caregivers, guiding them toward quality health outcomes.
Key Duties: Deliver patient-centered education, demonstrate effective organization and leadership, and possess knowledge of age-specific physiological, psychosocial, developmental, and spiritual needs.
Qualifications: 3-5 years hospital experience in a clinical setting, with preferred management and case management experience. Strong interpersonal, verbal, and written communication skills are essential.
Skills & Requirements: Ability to stand, walk, bend, and lift for extended periods; excellent speech, auditory, and visual acuity; familiarity with infectious materials; and possession of a valid CDE certification from the National Certification Board for Diabetes Educators.
