Your Role: Bilingual Community Health Worker (CHW)
Location: Anaheim, CA
Pay: $31.00/hourly
Your Toolkit Should Include:
- High school diploma or equivalent; related Associate’s or Bachelor’s degree preferred
- Bilingual in English and Spanish required
- Experience in community outreach, family engagement, peer support, or case navigation
- Strong communication skills and ability to work with diverse families and communities
- Basic computer and documentation skills
- Knowledge of and connection to the Anaheim community preferred
Your Superpowers Include:
- Connecting families with health, behavioral health, and community resources
- Providing outreach, health education, and family support
- Helping families navigate referrals, appointments, paperwork, transportation, and other barriers to care
- Building trusted relationships with families, residents, and community partners
- Participating in community events, workshops, playgroups, and outreach activities
- Tracking outreach, referrals, and program activities
- Supporting culturally responsive, family-centered services for families with children prenatal through age 5
