POSITION OBJECTIVE
:

The Case Manager will implement a person-centered, strengths-based approach to service delivery while assisting VEAP participants through the process of working towards stability in all facets of life. Additionally, the individual will continue to offer support and guidance as needed to ensure participants continue to maintain household stability. They will also play a role in creating awareness and advocate for participants’ housing rights.

RESPONSIBILITIES:

Provide assessment and case management at VEAP’s facilities and within the community to help participants obtain and maintain household stability to include housing through goal planning, coaching and education, resource and referral, and financial assistance.

Serve as an advocate and liaison between clients and property managers to develop a plan for participants to obtain and maintain household stability to include housing.

Coordinate services with other service providers and agencies to ensure participants get the necessary support to maintain household stability to include housing.

Provide basic translation and interpretive services for participants in both written and verbal formats.

Complete timely case notes and other record keeping requirements.

Represent VEAP at outreach events to inform the community of VEAP services and other resources.

Represent VEAP at community collaborative groups to increase awareness of housing barriers and partner with other organizations to address matters related to homelessness and affordable housing.

Assist volunteers in the lobby as needed providing support with all tasks to manage walk-ins and keep appointments on track.

Assist with responding to participant phone calls as needed.

Assist team with conducting intake over the phone, via e-mail and through letters, as needed.

Actively participate in VEAP’s continuous improvement initiatives by identifying and proposing opportunities to create efficiencies and expand services using a holistic approach.

Perform other duties as assigned.

EDUCATION AND EXPERIENCE
:

Required:

Bachelor’s degree (B.A./B.S.) from 4-year accredited college OR a 2-year Human Services degree with at least 2-years of relevant experience.

Proficiency in Microsoft Office (Word, Excel, and Outlook, TEAMs) and client databases.

Experience working with under-served or vulnerable populations, including persons from diverse racial, ethnic, and economic backgrounds, as well as with mental health and/or behavioral issues.

Demonstrated ability to professionally represent VEAP by maintaining composure and speak with tact and diplomacy.

Motivated self-starter with excellent problem-solving, time management, and organizational skills.

Detail-oriented, with demonstrated ability to work well under pressure and manage multiple projects and deadlines.

Exceptional written, verbal and interpersonal communication skills; excellent listening skills.

Ability to work with sensitive and/or confidential matters.

Able and willing to work evenings and weekends from time to time.

Preferred:

Bilingual in Spanish

Valid driver’s license.

Experience working with individuals and/or families experiencing homelessness or housing instability.
