What You’ll Do:
Provide supportive services to adults with mental illness living independently in the community. Services include community linkages, assistance with health and wellness related issues, and socialization opportunities.
Coordinate care with clinical teams and external agencies.
Help consumers sustain wellness and recovery beyond program enrollment.
Contribute to service improvements and consumer education.
Ensure compliance with licensing and regulatory standards.
Job Requirements:
Bachelor’s Degree in Social Work, Psychology, or related field from an accredited college or university.
At least 2 years of experience with community-based outreach services assisting mental health consumers preferred.
Strong computer skills (Word, Excel).
Valid Driver’s License and acceptable driving record.
Excellent communication, planning, and organizational skills.
