Social Worker – Caregiver Support Program | NYC (Hybrid)
We’re partnering with a well-established nonprofit organization in NYC seeking a Social Worker to support caregivers of older adults and make a meaningful impact in the community.
This is a hands-on, high-impact role working directly with individuals experiencing caregiver stress and burnout—helping improve outcomes for both caregivers and those they support.
Schedule + Setup:
• Hybrid: 3 days in office
• Must be open to travel throughout the NYC boroughs
What You’ll Do:
• Conduct intakes (in-person, virtual, and home visits) to assess needs
• Provide supportive counseling, caregiver education, and resource navigation
• Develop care plans to reduce stress and prevent burnout
• Screen for potential elder abuse and coordinate interventions
• Lead workshops and community outreach across NYC
• Maintain case records and track program outcomes (Salesforce a plus)
What We’re Looking For:
• MSW required (LMSW preferred)
• 2–3 years of experience in social services, case management, or similar
• Strong interpersonal, communication, and organizational skills
• Experience working with diverse communities is a plus
• Bilingual (Spanish, Chinese, or Russian) strongly preferred
Compensation + Benefits:
• $60,000 – $65,000 base
• Full benefits package (medical, dental, vision, 403b + more)
• Generous PTO + holidays
Location: NYC (Hybrid + borough travel required)
