The Amelia Ann Adams Whole Life Center (AAAWLC) is dedicated to promoting health, education, social justice, and healing within the San Joaquin County community. As part of our commitment to addressing the needs of our community, we are excited to announce an opening for a Sista Coach (Case Manager) for our Sista Coach program.
Project Description
The Sista Coach program at AAAWLC offers personalized support to pregnant and parenting Black/African American women, ensuring they receive culturally safe, comprehensive care throughout their pregnancy and until the baby’s first birthday.
Responsibilities
Conduct targeted outreach to enroll African American/Black mothers into the program.
Provide quality case management and care-coordination.
Collaborate with other Sista Coach programs and Black Infant Health teams.
Lead educational groups and coordinate program-related events and activities.
Complete grant and progress reports; develop success stories.
Attend collaborative meetings and assist with coalition building.
Lead program evaluations and assist in developing culturally relevant messaging.
Perform additional duties as assigned.
Desired Qualifications
At least two years of case management experience.
Experience serving Black parenting/birthing mothers.
Proven ability to develop and maintain strong relationships with a diverse group of stakeholders.
Demonstrated commitment to high standards and a history of achieving results despite challenges.
Passion for serving the community and a deep understanding of trauma-informed care.
Critical thinking and problem-solving skills.
Experience with outreach and case management.
Knowledge of trauma, Adverse Childhood Experiences (ACEs), and efficient strategies for support.
