The RN Clinical Educator & Case Manager – Private Care is responsible for supporting safe, high-quality, client-centered care through clinical education, competency validation, caregiver development, and RN case management for Private Care clients.
This position serves as the clinical education resource for Private Care caregivers and nurses, helping ensure associates are appropriately trained, competent, confident, and prepared to provide care consistent with the client’s individualized plan of care and scope of practice.
The RN Clinical Educator & Case Manager develops and delivers onboarding and ongoing clinical education, validates required competencies, identifies education needs through quality trends and field observations, and provides targeted remediation when performance or competency gaps are identified.
The position also provides RN case management for an assigned client population, including assessments, care planning, supervisory visits, clinical coordination, caregiver education, client/family communication, and escalation of changes in condition.
The role works collaboratively with Private Care leadership, scheduling, recruiting, People & Culture, Quality, and field associates to support caregiver readiness, client safety, regulatory compliance, retention, and exceptional client experience.
Responsibilities:
1. Clinical Education & Caregiver Development
Develops, coordinates, delivers, and evaluates clinical education for Private Care associates.
Responsibilities include:
- Develop and maintain a structured Private Care clinical orientation program.
- Provide clinical components of new-hire orientation for RNs, CNAs, HHAs, and other applicable caregivers.
- Ensure education reflects Florida regulatory requirements, organizational policies, scope of practice, client population needs, and Private Care service standards.
- Provide education using adult-learning principles, demonstration, return demonstration, simulation, case discussion, and field-based coaching.
- Develop education appropriate for both experienced caregivers and associates who require additional support transitioning into Private Care.
- Maintain standardized education materials, competency tools, skills checklists, and training records.
- Provide annual and recurring education based on regulatory requirements and organizational priorities.
- Coordinate education with Relias and other organizational learning resources when appropriate.
- Identify opportunities to improve caregiver confidence, clinical judgment, communication, and client service.
2. Competency Validation
Owns the clinical competency-validation process for Private Care associates within the scope assigned to the position.
Responsibilities include:
- Conduct or coordinate initial and ongoing competency assessments.
- Validate required clinical and practical skills through observation, demonstration, return demonstration, skills testing, or other approved competency methods.
- Ensure associates do not independently perform tasks for which required competency has not been demonstrated.
- Maintain complete documentation of competency validation.
- Identify competency deficiencies and establish remediation plans.
- Reassess competency following remediation when appropriate.
- Escalate competency or safety concerns to Private Care leadership.
- Partner with leadership and People & Culture when competency concerns may affect an associate’s ability to safely perform assigned responsibilities.
3. HHA/CNA Clinical Readiness
Supports consistent standards for determining clinical readiness of caregivers entering Private Care.
Responsibilities include:
- Review caregiver training and competency needs in partnership with Talent Acquisition and Private Care leadership.
- Participate in competency evaluation when an approved competency pathway is used to establish HHA qualification.
- Administer or oversee applicable written and practical competency assessments.
- Validate required practical skills before final clinical clearance.
- Ensure competency documentation is completed and forwarded for appropriate personnel-file retention.
- Identify candidates or associates who require additional education before being cleared for assignment.
- Provide targeted education for caregivers transitioning from other healthcare settings into Private Care.
The Clinical Educator provides the clinical competency determination; Talent Acquisition and People & Culture remain responsible for employment-related credentialing and onboarding processes as assigned by organizational procedure.
4. Client-Specific Caregiver Education
Ensures caregivers are appropriately prepared for the specific needs of the clients to whom they are assigned.
Responsibilities include:
- Review client care requirements and identify assignment-specific education needs.
- Provide or coordinate caregiver education before assignment when specialized knowledge or skills are required.
- Educate caregivers regarding client routines, safety risks, mobility needs, infection prevention, dementia-related needs, equipment, and other applicable care requirements.
- Provide additional education following hospitalization, significant change in condition, or change in plan of care when appropriate.
- Partner with scheduling to identify situations in which caregiver competency should be considered before assignment.
5. RN Case Management
Provides professional RN oversight for an assigned Private Care client population.
Responsibilities include:
- Complete initial and ongoing nursing assessments as required.
- Develop, review, and update individualized plans of care/service plans within applicable scope and regulatory requirements.
- Conduct required supervisory and reassessment visits.
- Evaluate client status, safety, functional needs, caregiver performance, and effectiveness of the care plan.
- Identify and respond appropriately to changes in client condition.
- Communicate clinically significant findings to the client, family, provider, Private Care leadership, or other appropriate parties.
- Coordinate care with physicians, healthcare providers, family members, community resources, and other members of the care team as appropriate.
- Ensure documentation is accurate, complete, timely, and reflective of the client’s current needs.
- Support continuity of care during transitions, hospitalizations, changes in caregiver assignment, and changes in condition.
6. Field Coaching & Performance Support
Provides visible clinical support to caregivers in the field.
Responsibilities include:
- Conduct field observations and coaching visits.
- Reinforce proper care techniques, documentation, infection prevention, safety practices, communication, and professional conduct.
- Provide immediate coaching when opportunities for improvement are identified.
- Distinguish between education/competency issues and performance/accountability issues and escalate appropriately.
- Develop individualized education or remediation plans when indicated.
- Conduct follow-up observations to verify sustained competency.
The goal is not simply to identify errors, but to reduce recurrence by addressing the underlying knowledge, skill, or competency gap.
7. Quality Improvement & Education
Uses quality data to drive the education program.
Reviews trends including:
- Client complaints
- Incident reports
- Falls
- Hospitalizations and emergency department utilization
- Caregiver performance concerns
- Documentation deficiencies
- Supervisory visit findings
- Competency failures
- Client/caregiver mismatches
- Infection-prevention concerns
- Medication-reminder concerns
- Safety events
- Service recovery trends
Develops targeted education based on identified trends and evaluates whether education results in measurable improvement.
Participates in QAPI or other quality initiatives as requested.
8. Orientation & New-Hire Support
Partners with Private Care leadership and Talent Acquisition to improve new-hire readiness and retention.
Responsibilities include:
- Provide standardized clinical orientation.
- Establish clear competency expectations before independent assignment.
- Identify new associates requiring additional support.
- Provide early field coaching when appropriate.
- Support structured 30-, 60-, and 90-day clinical development.
- Provide feedback regarding recurring recruiting, onboarding, or preparation gaps.
- Support successful transition from orientation to independent field practice.
9. Documentation & Compliance
- Maintain accurate education and competency records.
- Ensure competency documentation is completed within required timeframes.
- Maintain confidentiality of client and associate information.
- Support audit and survey readiness.
- Assist with regulatory surveys, audits, and corrective action plans as requested.
- Maintain current knowledge of applicable Private Care regulations, professional nursing standards, organizational policies, and scope-of-practice requirements.
- Identify potential compliance or patient-safety concerns and promptly escalate them.
Qualifications:
Education
- Graduate of an accredited school of professional nursing.
- Bachelor of Science in Nursing preferred.
Licensure
- Current, unrestricted Florida Registered Nurse license.
- Current CPR/BLS certification as required by organizational policy.
