We are seeking a dynamic and compassionate RN Clinical Coordinator/ Case Manager to join our dedicated healthcare team. Duties

  • Conduct comprehensive patient assessments, including vital signs, medical history, and physiological evaluations to determine appropriate care plans.
  • Develop and implement individualized case management strategies for diverse patient populations, including those with complex needs such as critical care, trauma, infectious disease, geriatrics, behavioral health, pediatrics, and post-acute care.
  • Collaborate closely with healthcare professionals across disciplines—physicians, therapists, social workers—to coordinate treatment plans and ensure continuity of care.
  • Manage discharge planning processes by evaluating patient needs for home health services, hospice care, outpatient clinics, or inpatient stays; facilitate timely transitions.
  • Utilize Electronic Health Records (EHR) systems to document patient information accurately while maintaining strict compliance with HIPAA regulations.
  • Oversee utilization management and medical record review to optimize resource use and ensure adherence to health regulation policies.
  • Provide patient service by educating patients and families about their conditions, treatment options, medication management, and follow-up care instructions.

Skills

  • Extensive critical care experience including ICU, emergency room, Level I & II trauma center experience, and hospital medicine.
  • Proven expertise in managing patients with complex conditions such as infectious diseases, geriatrics, behavioral health issues, and post-acute needs.
  • Strong knowledge of physiology, medical terminology, and clinical case management principles.
  • Proficiency in EMR/EHR systems for documentation and data management; familiarity with electronic health records management is essential.
  • Excellent patient assessment skills combined with the ability to communicate effectively with diverse healthcare teams and patients.
  • In-depth understanding of health regulation policies including HIPAA compliance and medical records management.
  • Experience in discharge planning, home health coordination, hospice care, outpatient clinics, and managing inpatient caseloads.
  • Ability to handle intake processes efficiently while managing multiple priorities in fast-paced environments such as emergency medicine or acute care settings.
  • Familiarity with managed care concepts and utilization review processes to ensure optimal patient outcomes while controlling costs.
  • Strong interpersonal skills for client communication and team collaboration; adept at managing patients as a nurse case manager across various clinical settings including pediatrics and geriatrics.

Pay: $48.00 – $52.00 per hour
