Our team of professionals, including a medical director, nurses, hospice aides, social workers, and volunteers, work in coordination with the patient’s physician to provide pain and symptom management, as well as social and spiritual support.
Summary
The Register Nurse is responsible for the delivery of patient care through the nursing process of assessment, planning, implementation, and evaluation. Identify and coordinate patient/family hospice care needs.
Responsibilities
Provide nursing care to patients and families utilizing the nursing process.
A thorough assessment, covering all aspects of the patient/family, including physical, emotional spiritual, social, psychological and cultural status should be obtained and documented appropriately.
Critical thinking skills should be applied to the analysis of the data collected.
Opportunities for improvement in palliation of symptoms should be identified and appropriate nursing diagnoses documented.
A plan of care is developed with interventions that adhere to the standards of hospice nursing.
The plan of care is developed with input from the patient/family and the interdisciplinary team.
The registered nurse is responsible for the communication and implementation of the plan of care in all settings.
Ongoing evaluation of progress toward appropriate outcomes is done and documented.
Reassessment and revision of the plan of care is completed as needed in collaboration with patient, family, and interdisciplinary team and documented.
Required to maintain current knowledge and competency of hospice nursing.
Contributes to an environment conducive to educating and welcoming nursing students, other healthcare students, and other members of the interdisciplinary team, as appropriate.
Qualifications, Education and Experience
Successful completion of a Registered Nurse program through an accredited institution.
BSN strongly preferred.
One (1) year of medical/surgical experience with Hospice, Oncology, Home Health/Private Duty experience being preferred.
Basic computer skills.
Knowledge of acceptable RN nursing practices and procedures.
Demonstrated knowledge of universal precautions.
Demonstrated ability to communicate effectively.
Current licensure as an RN in the State of Kansas.
Valid drivers’ license.
Proof of current auto insurance.
