Position: Case Manager – Booth Haven Men’s Shelter – Rochester, NY

Part Time (This is a 25 hour/week position – 5 hours/day)

Monday, Tuesday, Thursday and Friday 7:00 a.m.- noon, and Wednesdays 10:00 a.m.-3:00 p.m.)

Rate: $22/hour – Non-Exempt position

SCOPE AND PURPOSE OF POSITION:

Under the direct supervision of the Program Manager, the Case Manager provides case management services to the Men’s Shelter clients during their temporary stay at Booth Haven. Assists in oversight of Resident Assistants.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Assess and identify clients’ needs. Advocate for clients to ensure that services of referral are accessible and provided to clients.

2. Develop an individualized service plan for each resident and ensure periodic review of goals.

3. Provide supportive and crisis counseling and guidance both individually and to groups.

4. Provide information and refer clients to relevant community social services for medical, mental health, substance abuse, educational, vocational, public assistance, etc.

5. Maintain all records and documents related to the interview/counseling meeting and subsequent progress/status reports in accordance with agency and contract requirements.

6. Develop and maintain effective working relationships with other community social service providers (e.g., Dpt. of Human Services, Veteran’s Administration, etc.) to enhance the quality of services to clients. Represent the program in the community as requested and as approved.

7. Report observations to supervision on trends of the homeless, their service needs, effectiveness of referrals, etc.

8. Maintenance of Booth Haven database by inputting daily data for DSS billing information and record keeping.

9. Maintenance of HMIS data entry (intakes, transfers, discharges, etc.)

10. Compile monthly statistics and prepare the required reports in a timely manner.

11. Conduct Case Management related client groups either directly or co-facilitate with community organizations.

12. Collaborate with staff for case management and programmatic services.

13. Share 24-hour on-call responsibilities with the Program Manager and Senior Resident Assistant once every three weeks.

14. Attend staff meetings, in-service conferences, seminars, training programs, and other agency informational meetings as required.

15. Complete all mandatory training (in-person and online) in a timely manner.

16. Share office coverage at least 1-2x a week during lunch time.

17. Other duties as assigned.

SPECIAL SKILLS, CERTIFICATES, LICENSES, REGISTRATIONS.

1. Bachelor’s degree (associate’s degree considered with experience) in human services preferred (Social Work, Psychology, Sociology, Criminal Justice, etc.) with 2 years’ case management experience required

2. Requires experience working with and providing advocacy for vulnerable populations, as well as an understanding of barriers faced by those who are homeless

3. Concern and sensitivity toward vulnerable populations

4. Knowledge of social service resources

5. Must be proficient with Microsoft Office 365 suite of applications and other standard data collection systems

6. Ability to collaborate effectively with external service providers

7. Must possess strong organizational abilities and attention to detail

8. Excellent communication and problem-solving skills

9. Valid New York State Driver License for transport of residents
