This is a full-time position working 8 individuals with developmental disabilities and medica needs. This position oversees a large team that includes DSP’s and nurses.

The Program Supervisor supports those in our care to reach their highest potential by providing them with assistance to achieve their goals and their daily activities.

The essential function of the position includes the following:
- Supervises, recruits, trains, assigns, monitors and evaluates the work of all shift nurses and Therapy Aid staff under the supervision of the Program Manager.
- Fills in open shifts as needed.
- Prepares, posts and makes needed adjustments to staff schedules; keeps the Program Manager informed of changes.
- Coordinates, arranges and completes provisions for all ancillary services as identified in the individual habilitation plan and program participant needs as they occur.
- Participates in the initial assessment of new referrals, including coordination of identified consultations.
- Ensures Therapy Aide staff provide services to the program in accordance with the Principles of Normalization through role modeling, training and supervision.
- Implements 90-day individual assessments and individual habitation plans which include goals (both formal and informal), service objectives and behavioral plans of support.
- Responsible for pre-training, evaluations and on-going supervision of the ICF/ID staff as a means of monitoring work standards and individual progress.
- Monitors social and recreational activities, both at the residential program and within the community.
- Responds appropriately to emergency situations, including notification and documentation.
- Ensures availability and coordinates transportation for program participants to day program and recreational activities.
- Attends all meetings and trainings as required. Conducts staff meetings and trainings as necessary.
- Guides and assists program participants in learning to live and function as independently as possible within the community.
- Assists in direct care of program participants and in routine tasks necessary to provide support services to program participants, i.e. meal preparation and clean-up, household cleaning, laundry, bed-making, etc. Provides a safe and secure environment for program participants both within the site and while driving the Agency vehicle.
- Lifting: Must be physically capable to assist in lifting a person up to 250 pounds, i.e.: during bathing, toileting, seizures, in the event of accidental falls, etc. and independently be capable of lifting a minimum of 20 pounds, i.e. groceries, laundry, etc.
Location of available shifts: Our homes are located in Nazareth, PA.
Available shifts: Hours will vary to meet the needs of the individuals and the program participant. Every other weekend is a MUST. Including but not limited to evenings, overnights, weekends, and holidays.
All Team Members must:
· be 18 years of age or older
· have a current driver’s license with no suspensions or points within the past three years
· Bachelor’s degree from an accredited college or university and a minimum of 2 years direct care developmental disability experience; or Associate’s degree/60 credit hours from an accredited college or university and a minimum of 4 years direct care developmental disability experience, or a minimum of 1 year supervisory experience and 2 years direct care developmental disability experience; or a High School diploma/GED equivalent and a minimum of 6 years of direct care developmental disability experience or a minimum of 1 year supervisory experience and 3 years direct care developmental disability experience.
· experience working in full care sites providing personal care to individuals in services (preferred)
· have no prohibitive criminal offenses
· be available to work on weekends and holidays
