Northridge Hospital Medical Center
Under the direction of the Director of the Center for Healthier Communities, the Program Coordinator is responsible for coordinating, supervising, implementing and evaluating programs services and activities for the Center for Healthier Communities for multiple grant funded projects. These programs focus on areas including chronic disease management and prevention and abuse prevention programs. This position is also responsible for program development and grant writing.
Responsibilities
Responsible for the development and implementation of CHC grant funded programs including the California Department of Public Health Prevention Forward grant and California Office of Emergency Service Sexual and Domestic Violence Prevention Grant.
Supervises achievement of deliverables and leads team members for the CHC programs.
Facilitates workshops, classes, and groups to educate community members for CHC programs.
Develops effective outreach programs for CHC programs.
Responsible for grant writing for CHC projects and new initiatives.
Researches funding opportunities for CHC programs.
Develops materials including curricula and workshops for community-based classes.
Evaluates effectiveness and efficiency of CHC programs and makes adjustments to increase effectiveness
Represents CHC before community based organizations, governmental and policy organizations.
Performs other related duties as assigned or requested.
Ability to work occasional nights and weekends.
Qualifications
Experience managing community-based programs, abuse prevention, ability to create multi-disciplinary team, grant reporting, and project management. Experience creating and presenting workshops and evidence-based curriculums.
Bachelor’s Degree (BA/BS) required.
Master’s degree preferred.
Current Fire Safety card required. ( CT FIRESAF )
Bilingual Spanish preferred.
Special Skills
Basic knowledge of Excel/Sheets, Zoom and Canva. Presentation and public speaking skills.
CHES OR CPH certificate preferred

Pay Range
$34.50 – $45.21 /hour
