Position Summary

The Case Manager I is a high demand position that provides holistic, strength-based, harm reduction case-management services specializing in housing. Responsibilities include removing barriers to housing readiness, assisting guests with appointments, coordinating housing documents, developing housing plans, creating budgets for sustainable housing, linking guests to resources, applying for housing, and housingadvocacy. The Case Manager I also participates in weekly Coordinated Entry System (CES) housing match meetings to provide updates on individuals.

Essential Functions

Case Management:

Ensure holistic, strength-based, harm reduction case management is provided in a timely, efficient, and effective manner.

Maintain an active case load.

Work with guests to remove barriers to housing.

Create case plans with each guest and perform needs assessments.

Meet with each guest on case load four times a month.

Maintain the case load tracking system.

Ensure guest case notes are entered weekly in databases.

Assist guests with obtaining necessary documents for housing.

Schedule and facilitate weekly life skills groups related to housing needs.

Utilize conflict management and crisis intervention techniques as needed.

Housing Navigation:

Develop individualized guest-centered housing plans.

Conduct visits to maintainguest engagement.

Research and visit housing options for low-income individuals/seniors.

Maintain a list of current housing vacancies.

Assist guests in locating and securing permanent housing.

Create exit plans with each guest.

Facilitate training for new hires, interns, and volunteers.

Record Keeping:

Maintain documentation of housing documents, guest goals, and reporting.

Enter guest case notes into databases in a timely manner.

Exit guests from case load and databases.

Ensure accuracy in billing submissions.

Communicate guest status with supervisors and other agencies.

Miscellaneous

Participate in programming and housing case management meetings.

PAY RATE: $25.00/-$27.00/hr.

Working Conditions

Ability to walk, stand, bend, squat, climb, kneel, and twist on an intermittent or continuous basis. Ability to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead. Ability to operate computer, fax, and telephone. Ability to lift up to 40 lbs.

Minimum Qualifications

Understanding of The Salvation Army’s Christian mission.

BSW or equivalent degree (preferred).

Minimum 1-year experience in case management with recovery, mentally ill, and/or homeless individuals.

1-year experience with HMIS (preferred).
