Job Overview

The Patient and Family Education Coordinator is a Registered Nurse (RN) working on the Health Literacy team or assigned departmental team(s). The Patient and Family Education Coordinator serves as a clinical expert on patient and family education across the Children’s Hospital Colorado system of care and/or within assigned area.

Qualifications

Education: Bachelor of Science in Nursing (BSN)

Experience: 3 years of clinical pediatric experience or previous educator experience.

Certification(s): BLS/CPR from the American Heart Association. Achievement and maintenance of an American Nurses Credentialing Center (ANCC) or Magnet approved certification.

Licensure(s): Current Colorado RN License

POPULATION SPECIFIC CARE

Neonate – <30 days Infancy - >30 days to 1yr

Toddlers – >1yr to 3yrs

Pre-Schoolers – >3yrs to 5yrs

School age – >5yrs to 13yrs

Adolescent – >13yrs to 18yrs

Adult – >18yrs to 65yrs

ESSENTIAL FUNCTIONS

An employee in this position may be called upon to do any or all of the following essential functions. These examples do not include all of the functions which the employee may be expected to perform.

Collaborates with patients or caregivers and interdisciplinary teams to develop, provide, and evaluate patient or caregiver education based on individual learning needs and barriers.
Utilizes clinical expertise and health literacy best practices to provide recommendations on patient education resources. Collaborates with patients, caregivers, and other care team members across the continuum of care with quality improvement and research activities, impacting care in designated program.
Coordinates Family Learning Center or departmental programming by: updating class curricula and teaching materials based on policy updates and health literacy best practices; orienting and supporting other RNs to teach the classes; supporting scheduling of patients or caregivers for classes; providing direct patient or caregiver education.
Utilizes policies, procedures, and clinical care guidelines to provide patient or caregiver education that improves patient outcomes.
Optimizes and maintains specialty related tools and resources to meet the needs identified by interdisciplinary teams.
Facilitates inter-departmental communication through their role as a liaison on house-wide committees and in collaboration with colleagues on other units.
Develops and delivers presentations and other material for use during program-related events and meetings. Provides training and support to staff in providing standardized education and documentation.
Develops relationships with other pediatric organizations sharing similar programming, to collaborate and share data that supports improved patient outcomes. Participates in professional organizations focused on health literacy, population health and/or assigned practice area.
Maintains data systems to track interventions and outcomes related to patient or caregiver education.

Guidelines: Guidelines are generally numerous, well established, and directly applicable to the work assignment. Work assignment and desired results are explained by general oral or written instructions.

Complexity: Duties assigned are generally repetitive and restricted in scope but may be of substantial intricacy. Employee primarily applies standardized practices.

Decision Making: Decisions or recommendations on non-standardized situations are limited to relating organizational policies to specific cases. Brings non routine issues to supervisor. Problems that are not covered by guidelines or are without precedent are taken up with the supervisor.

Communications: Contacts with the public or employees where explanatory or interpretive information is exchanged, gathered, or presented and some degree of discretion and judgment are required within the parameters of the job function.

Supervision Received: Under normal supervision, within a standardized work situation, the employee performs duties common to the line of work without close supervision or detailed instruction. Work product is subject to continual review.

Salary Information

Annual Salary Range (Based on 40 hours worked per week): $75,957.07 to $113,935.61
Hourly Salary Range: $36.52 to $54.78
