Position Summary:
We are seeking a detail-oriented and compassionate Patient Care Scheduler to join our hospice care team. This vital role focuses heavily on scheduling coordination and ensuring patients receive timely, high-quality nursing visits in alignment with their care plans. As the primary point of contact for clinical scheduling, you will work closely with nursing staff, CNAs, and other departments to manage visit logistics, maintain patient records, and support operational efficiency. Your work will have a direct impact on the quality of care our patients receive during their most critical time.
JOB FUNCTIONS:
Coordinate and schedule nursing visits to ensure timely and appropriate patient care.
Maintain and update visit schedules daily in EMR, collaborating with clinical teams to align staffing and patient needs.
Track and maintain a daily visit verification log; resolve scheduling gaps or discrepancies proactively.
Communicate with nurses and CNAs to ensure coverage and continuity of care.
Assist CNA Coordinator with follow-ups, as needed.
Maintain patient charts with all DME equipment in use with patient(s).
Assist with all incoming office phone calls with the ability to problem-solve and resolve matters.
Maintain daily reminder report listings to ensure it is updated with new patients and reflect the removal of deceased patients.
Initiate and process requests for DME pickups after patient deaths.
Responsible for communicating updates to clinical teams pertaining to the daily ADP list.
Complete and document calls with new patients to discuss the services to be provided.
Monitor the EMR daily to ensure visits are complete and synced by 10 am.
Maintain patient visit daily/weekly count per clinician to determine if standards are met.
Maintain a comprehensive working knowledge of hospice relationships with hospitals, assisted living facilities, and others.
EDUCATION:
High School Diploma or GED required; advanced education preferred.
WORK EXPERIENCE:
Prior experience in a health care environment, home care, or hospice preferred.
Knowledge of medical terminology preferred.
SPECIAL SKILLS:
Exhibit a competent working knowledge of the following computer software programs: Microsoft Word and other basic Windows applications.
Exhibit excellent interpersonal, organizational, written, and verbal communication skills.
REQUIREMENTS:
Must have good communication skills and techniques with facilities, patients, families, office staff, and physicians.
