Schedule: Monday-Friday, 8a-5p

The PATH Coordinator helps individuals experiencing housing challenges connect with resources, services, and community supports that promote housing stability and positive outcomes. This community-focused role builds relationships with consumers, landlords, service providers, and community partners while helping individuals navigate housing and other essential supports. Working from a Housing First and low-barrier approach, the PATH Coordinator meets people where they are and helps create meaningful pathways toward greater stability and independence.

Key Responsibilities

  • Connect individuals with housing programs, community resources, benefits, and supportive services that promote stability and independence
  • Conduct outreach and respond to referrals for individuals experiencing housing challenges or homelessness
  • Coordinate with internal teams, Continuum of Care partners, and community organizations to support housing goals
  • Build and maintain relationships with landlords and housing partners to expand housing opportunities
  • Support individuals with navigating appointments, benefits, SOAR, and other services that contribute to overall stability
  • Provide coaching and practical support related to independent living skills and maintaining housing
  • Participate in community housing initiatives, coordinated entry activities, case conferencing, and local events
  • Maintain accurate documentation of program and grant activities, including HMIS
  • Provide services in office, community, client, and field-based settings as needed


Requirements, Skills, Knowledge and Expertise

  • High School/GED required
  • Bachelor’s degree in social work, sociology, psychology or related field preferred
  • At least four (4) years’ experience or a combination of education/experience in human services required if Bachelor’s degree is not obtained
  • A valid, unrestricted driver’s license at the time of hire with the ability to obtain and maintain a class E MO driver’s license by the end of 60-day introductory period required

Must obtain and maintain current SOAR certification within 60 days of hire

  • Bring a compassionate, strengths-based approach to working with people and communities
  • Are comfortable meeting people where they are and supporting self-determined goals
  • Build relationships easily and communicate effectively with individuals from diverse backgrounds
  • Are resourceful, persistent, and creative when navigating complex systems or limited resources
  • Can balance independent work with collaboration across clinical, community, and housing partners
  • Have an understanding of or interest in Housing First, low-barrier services, and person-centered support
  • Want your work to have a meaningful impact on individuals, families, and rural communities
