PART TIME RE-HOUSING CASE MANAGER POSITION Jobs in Falmouth, MA at Belonging to Each Other, Inc.
Belonging to Each Other (also known as BTEO), a Falmouth Massachusetts nonprofit, takes a holistic approach to addressing homelessness in Falmouth- one that goes beyond providing temporary beds to offering consistent support that leads to long-term stability.
We are adding a year-round, part-time case manager to our team. The job requires 15-21 hours per week, on a flexible schedule, with most hours spent Monday through Friday, between 9:00am and 5:00pm. This is an in-person position, with most work occurring in and around Falmouth Mass.
BTEO provides short-term programs in two residences (each housing 8-12) where adult women and men experiencing homelessness can call home temporarily. There they regroup and work on a goal to move to stable housing within 6 months. BTEO accepts up to 19 women and men (in roughly equal numbers) at the beginning of each program. When a program participant achieves stable housing and moves on, a new person is accepted into the program.
· You would work face to face or by telephone or video with 7-9 program participants of both genders. You will assist residents to re-connect with available community resources for improved health, nutrition, transportation, employment and to prepare for tenancy (learn to budget, locate housing options, prepare or update applications with documentation).
Historically, most people in each session leave the program for an apartment, a shared home, or a longer-term residential program appropriate for them. BTEO continues to provide follow-up assistance to former residents after they leave the residence. We provide in-community services to these former residents as needed, to help them adjust to their new situation.
· You would also call or meet with some of these former residents to check in (as assigned) and help address issues. Often, these meetings could be held before or after an existing lunch program open to people who need support.
People are referred to our programs by outreach workers from local organizations, including the Town’s Human Services Department, social workers from healthcare agencies and outreach workers from other agencies with which we work closely. BTEO meets regularly with representatives from these organizations to facilitate referrals and advocate for services for former residents who may be falling through a gap in existing services.
· You would participate in person or via Zoom in those meetings (3-4 hours per month), as our representative.
Qualifications for the Position
· Relevant education and/or experience in human services or related field. Associate’s degree preferred, not required.
· Minimum of one year of professional experience working with people experiencing homelessness or other at-risk populations.
· A valid driver’s license required, and use of a registered vehicle.
· Ability to travel and drive as needed, including occasionally to locations across Cape Cod.
· Ability to be supportive and respectful to all people served.
· Capability to work both independently and as part of a team.
· Ability to collaborate with other BTEO staff members and with external referral sources and partners, including [Housing Assistance Corp, VinFen, State Agencies, the Falmouth Service Center, Town of Falmouth staff and Falmouth’s Housing Authority and Housing Corporation.
· Possess written and oral communication skills in English, with documentation abilities.
· Demonstrate a level of understanding and comfort with various forms of technology, including data entry and reporting systems.
· Ability to take direction and participate in regular supervision.
Organization Overview
BTEO () is a community-funded nongovernmental nonprofit organization in Falmouth, MA.
BTEO accepts Falmouth-connected adults into the program described above who agree to BTEO’s code of conduct that includes mutual respect, abstaining from substance use while in the residential program, abide by program rules, and working with their case managers toward obtaining more stable housing.
Individuals eligible for our programs are screened by our Program Director, and applicants for whom the BTEO program is not appropriate are referred to other appropriate community resources.
Case managers collaborate with their assigned people on a housing stability action plan, addressing each person’s impediments to attaining stable housing. Similar case management services are provided to former program participants from time to time to help them address issues that may arise affecting their housing stability.
BTEO’s 115 active volunteers assist the program by preparing, supplying and maintaining the residences, providing food for special occasions, helping with enrichment and skill-building activities, and transporting residents to work and appointments. Volunteers are screened and supervised by our staff Volunteer Coordinator. Volunteers are assigned to our residential program under the direction of the Program and Clinical Director.
Summary of Responsibilities
· Help adult individuals selected by BTEO make progress on an action plan to find stable housing and/or social services.
· Develop and maintain a knowledge of social service and housing resources on Cape Cod, using information and resources provided by BTEO and other local agencies.
· Work independently, transparently and collaboratively as part of a team that includes a Program and Clinical Director, the Executive Director, other case managers and key volunteers.
· Work with program participants to develop action steps, based on the Clinical Director’s evaluation of the status of people assigned to you and the factors affecting housing stability, prioritizing those factors most likely to be positively impacted by using BTEO time and resources.
· Work with each assigned person by phone, video conference, or in-person at least weekly and document your work and key milestones in the person’s electronic file (we use a case management app that you will be trained on).
· Visit the BTEO sites in person in Falmouth at least twice a week for meetings with residents, and check-in by phone, text or email with your assigned residents at least once every 48 hours to encourage progress towards the action plan.
· Case conference on a weekly basis at a mutually agreed time with the other members of the case management team; prepared to report on progress by each person assigned to you.
· Maintain professional composure in stressful situations.
· Strictly maintain confidentiality of current and former program participants.
· Communicate clearly, solve problems and prioritize.
Work Environment
BTEO’s sites are in Falmouth, MA, and work will be performed in and around the Upper Cape, including the BTEO office in Falmouth and rental houses in neighborhoods in downtown Falmouth. BTEO sites have internet capability. You are expected to have your own transportation to worksites but are eligible for a monthly vehicle allowance and reimbursement for travel out of Falmouth.

Pay: From $21.00 per hour
