About the role
The Outreach Case Manager role is a 0.9 position, providing outreach-based case management to participants exiting custody with multiple complex needs for a period of one to nine months. This role requires working closely with key stakeholders in particular Corrections Victoria, Community Correctional Services (CCS), prisons and other external organisations. The role includes frequent travel to Victoria’s ‘mens’ prisons and includes provision of day of release transport from regional locations.
We are looking to recruit an enthusiastic and experienced Case Manager who is able to provide case management to men and genderdiversepeople exiting prison fromdiversebackgrounds.
