Role Description
This is a full-time remote role for a Nurse Case Manager-RN. The Nurse Case Manager-RN will be responsible for coordinating and managing patient care, including admissions, discharge planning, utilization management, and case management. Daily tasks will involve assessing patient needs, developing care plans, coordinating with healthcare providers, and ensuring patients receive appropriate medical and nursing care.
Qualifications
Experience in special needs/ complex care Nursing
Skills in authorizations and appealing
Excellence in Case Management
Excellent communication and organizational skills
Ability to work independently and remotely
Experience in working with medically complex and fragile children is a plus
Bachelor’s degree in Nursing (BSN) and an active RN license
