Emerald Trace is seeking an experienced RN or LPN MDS Nurse to join our team in a full-time position.

What You’ll Do
As our MDS Nurse, you’ll play an important role in ensuring resident assessments are accurate, timely and compliant while helping our interdisciplinary team appropriately capture each resident’s needs and services.
Your responsibilities will include:
- Coordinating and completing MDS assessments in accordance with CMS requirements
- Managing components of the Resident Assessment Instrument (RAI) process
- Ensuring assessments are completed accurately and within required timeframes
- Reviewing clinical documentation for accuracy and completeness
- Supporting accurate PDPM reimbursement
- Working closely with nursing, therapy, social services, dietary and other members of the interdisciplinary team
- Participating in resident care planning and care conferences
- Helping coordinate resource utilization and reimbursement
- Maintaining compliance with federal, Kentucky and facility requirements related to MDS and resident assessment
What We’re Looking For
Required:
- Current Kentucky RN or LPN license
- Previous MDS experience in a skilled nursing/long-term care setting
- Long-term care experience
- Working knowledge of the MDS/RAI process
- Strong organizational and time-management skills
- Excellent attention to detail
- Ability to manage deadlines and multiple assessments
- Strong communication and interdisciplinary teamwork skills
Preferred:
- 3+ years of MDS experience
- Strong working knowledge of PDPM
- Experience with Medicare skilled services and long-term care reimbursement
- Knowledge of CMS and Kentucky skilled nursing requirements
- Experience with electronic clinical documentation and MDS software
Compensation & Benefits
Up to $43.00 per hour based on experience, qualifications and applicable incentives.
