How you’ll make an impact in this role
- Direct all aspects of our Social Systems Initiatives (SSI) program, which focuses on identifying and responding to the social and health needs of individuals and communities at the highest risk for poor health outcomes
- Define program goals and infrastructure to align with business development strategies and partnerships.
- Direct the program team to achieve goals while providing orientation, training, coaching, and mentoring. This includes completing performance evaluations and handling disciplinary actions.
- Create program plans, management tools, and reporting capabilities.
- Develop the program budget and monitor or adjust plans as resource requirements change.
- Serve as the primary liaison between programs, internal departments, and external partners.
What Minimum Requirements You’ll Need
Licensure / Certification / Registration:
- Project Mgmt Professional preferred. Licensure required relevant to state in which work is performed.
Education
- High School diploma equivalency with 5 years of applicable cumulative job specific experience required, with 2 of those years being in leadership/management OR Associate's degree/Bachelor's degree with 3 years of applicable cumulative job specific experience required, with 2 of those years being in leadership/management.
What Additional Preferences We're Seeking
- Bachelor of Science in Nursing (BSN) with an active Registered Nurse (RN) license or Licensed Clinical Social Worker (LCSW) preferred.
- Previous experience with Maternal Navigation and healthcare delivery systems, including the ability to identify and determine barriers to care.
- A collaborative and transformational leadership style across the community of practice and with key stakeholder groups.
- Data-driven approach with the ability to apply a systems perspective to navigate and resolve complex issues.
- Proficiency in understanding and implementing the Social System framework.
- Strong program and project management experience.
