Role Description
The Intensive Case Manager is a part-time, hybrid role based in Albuquerque, NM, with a combination of on-site work and some work-from-home flexibility. This role provides intensive case management services to women with co-occurring mental health and substance use disorders, including needs assessment, service planning, and coordination of resources such as housing, healthcare, education, and employment support. Day-to-day responsibilities include maintaining a manageable caseload, conducting in-person and virtual meetings, advocating for clients within social service and criminal justice systems, and supporting clients in rebuilding family and community relationships. The Intensive Case Manager will document services in a timely and accurate manner, collaborate with counseling staff and community partners, and participate in team meetings and trainings to ensure trauma-informed, integrated care. The role requires professional boundaries, cultural humility, and a commitment to supporting client-centered recovery and empowerment.
Responsibilities:
- Conduct program participant screenings and intakes.
- Ensure that participants are enrolled in appropriate Federal and State programs, and are referred to appropriate community resources.
- Meet with program participants a minimum of once per week via home visits, office visits, telephone, email, texting or virtually. An in-home visit is required every 30 days.
- Support housing placement, housing application processes, and assistance with move-in activities.
- Prepare notes and enter billing in a timely manner in accordance with agency and funder requirements.
- Maintain participant files and ensure compliance with funder requirements.
- Attend staff meetings, trainings, and supervision meetings with direct supervisor.
- Carry cell phone and provide crisis intervention as required.
Requirements:
- Demonstrated experience in Case Management Services, including assessment, service planning, and resource coordination.
- Background in Social Work and Social Services, with knowledge of community resources, public benefits, and systems that impact women with co-occurring disorders.
- Educational or practical experience in Psychology, especially related to mental health, trauma, and substance use recovery.
- Ability to work effectively with individuals who have complex histories, including justice involvement, family separation, and housing instability.
- Strong interpersonal, communication, and documentation skills; ability to maintain accurate records and meet reporting requirements.
- Commitment to gender-specific, trauma-informed, and culturally responsive care; ability to establish trust while maintaining professional boundaries.
- Required: High School diploma and certification in CCSS and/or CSW.
- Preferred: Bachelor’s degree in Social Work, Psychology, Counseling, or related field; relevant licensure or certification is an asset.
- Reliable transportation for local travel in Albuquerque, NM; willingness to work part-time daily from 1:00 pm to 4:00 pm.
Compensation:
- $28 – $30 per hour
- Paid time off
- Seven paid holidays per year
Pay: $28.00 – $30.00 per hour
