Job Title: Intake Referral LPN

Education/Experience:
· LPN License required
· 2 years’ experience in home health or intake preferred
· Excellent computer skills
Job Summary:
The Intake Referral Nurse assists in qualified referral acceptance, insurance verification, and manager care authorization management.
Job Tasks:
· Submits requests for prior authorization/ongoing visits for all managed care patients.
· Monitors all managed care activity on a weekly basis for authorizations, denials, date extensions, retro visits, etc.
· Verifies continued insurance coverage for all patients on a monthly basis.
· Obtains SOC verbal orders for facility referrals.
· Verifies insurance coverage
· Monitors electronic referral sources
