For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, currently offering 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness.
Our Home Base HomelessnessPreventionPrograms provide services to those who are at risk of entering a shelter due to rental arrears, overcrowding, or substandard housing, and to those who have a prior shelter history as part of aftercare services. Services may include case management, tenant/landlord mediation,advocacy/benefit entitlements, short-term financial assistance, referrals, and varied workshops that can help individuals who are at risk of homelessness.
STATEMENT OF THE JOB:
Under the direction of the Supervisor, the Intake Coordinator helps the program meet its enrollment target and assists in pre-screening, intakes, reviewing case files, and assigning cases.
Review case files and client database for proper documentation at initial intake and at intervals as required by funding stream.
Conduct prescreens and intakes and create client service plans.
Assign cases.
Provide interventions in the area of diversion, aftercare, andprevention.
Review requests for financial assistance.
Ensure that referrals are made to appropriate internal and external services as required to meet the consumer’s/family’s needs.
Follow-up on referrals made on behalf of a consumer/family and respond to all referrals made to you/your program.
Provide support to theQuality AssuranceSpecialist.
Assist supervisory staff to monitor intakes and ensure pending cases are completed within required time frames.
QUALIFICATIONS:
MS/MA degree or BS/BA with equivalent experience (2 years).
Experience in social service setting and providing direct services to homeless individuals/families, or at-risk populations defined as vulnerable adults, families, and youth, including but not limited to those with a history of or involvement with child welfare, addiction, physical or emotional issues, unemployed/under-employed desired.
Bilingual skills (Spanish) a plus.
Ability to workflexible hours, including weekends and evenings according to the needs of program operation.
Able to conduct home/field visits and travel throughout the five boroughs as needed.
Frequently lifts and/or moves up to 10 pounds.
