The Road Home operates multiple resource centers that provide 24/7 emergency shelter, basic needs such as beds, showers, clothing, and hygiene items, and connections to critical services. Case managers work closely with priority populations, including families, Veterans, and people experiencing chronic homelessness, to create individualized plans and access benefits, healthcare, and employment support. Once clients are housed, The Road Home continues to provide in-home case management to help them maintain housing, increase income, and overcome barriers to long-term stability.
Role Description The Housing Focused Case Manager is a full-time, on-site role based in Salt Lake City, UT. This role provides intensive case management services to individuals and families experiencing homelessness, with a primary focus on helping them obtain and maintain stable housing. Day-to-day responsibilities include conducting housing and basic needs assessments, developing and monitoring individualized case plans, and supporting clients in securing appropriate housing options. The Case Manager coordinates referrals to community resources, including public benefits, substance use treatment, mental health services, employment and job training programs, childcare, and other supportive services. The role also involves regular in-person and in-home visits to ensure lease compliance, address housing-related issues, and help clients build skills that promote long-term housing stability and increased self-sufficiency. The Housing Focused Case Manager collaborates with internal teams, landlords, and partner agencies, maintains accurate documentation in agency databases, and adheres to safety, confidentiality, and trauma-informed care practices.
Qualifications

  • Experience in case management, social work, or related human services roles, including assessment, service planning, and client advocacy.
  • Knowledge of housing resources, homelessness services, and community-based support systems, particularly within Salt Lake County and the Wasatch Front.
  • Strong communication and interpersonal skills, with the ability to build trusting, professional relationships with diverse clients, landlords, and partner agencies.
  • Solid organizational and time management skills to manage caseloads, maintain accurate records, and meet program deadlines and reporting requirements.
  • Ability to conduct on-site and in-home visits, navigate crisis situations, and apply trauma-informed, strengths-based, and culturally responsive approaches.
  • Proficiency in basic computer applications and case management or data tracking systems used for documentation and reporting.
  • Relevant education such as an associate’s or bachelor’s degree in social work, psychology, human services, or a related field; equivalent experience may be considered
