Join our compassionate team as a Hospice Chaplain/Spiritual Counselor and play a vital role in providing spiritual support, comfort, and guidance to patients facing terminal illness and their families. Functions

  • Serve as a member of the hospice interdisciplinary team (IDT)
  • Complete spiritual care assessment for patients admitted to hospice care within four days of receiving referral from admitting RN Case Manager
  • Contribute to and update hospice plan of care
  • Provide direct spiritual support and/or counsel to patients/families in keeping with patients’/families’ beliefs
  • Work with hospice staff and clergy to enhance their sensitivity to the spiritual concerns of patients/families experiencing terminal illness and loss
  • Participate in IDT meetings by exploring and assessing the potential spiritual needs of patients/families and reporting on services as indicated
  • Maintain proper documentation of visits to patients/families
  • Make contact with clergy or appropriate representatives of patients/families as indicated
  • Conduct or make arrangements for funeral or memorial service when requested

Experience

  • Proven experience working as a clergyperson or spiritual counselor within a hospice, healthcare, or nonprofit faith-based organization setting
  • Demonstrated ability to work effectively with culturally diverse communities and families facing terminal illness
  • Expertise in pastoral theology, spiritual counseling, crisis intervention, and terminal illness counseling
  • Excellent written communication skills for documentation, reports, and educational materials
  • Experience mentoring others in spiritual or pastoral settings while managing conflict or crisis situations with sensitivity

Pay: $25.00 – $28.00 per hour
