Job Description
We are seeking an EITP Case Manager to join our team! Previous experience in HIV and AIDS. EITP stands for early intervention treatment program. We try to get patients back into care and back on their medication.
Essential Duties and Responsibilities:
Case Management:
Client intake
Client comprehensive assessment
Individual service plan development
Implementation of service plans, follow-up and reassessment
Acuity level reduction
Case closure
Complete a monthly summary of specific case management data that is needed for Data Report.
Assist clients with completing Aids Drug Assistance Program forms for enrollment and recertification.
Assist in preparation of quarterly and annual reports.
Make clinic appointments for EITP clients through the clinic appointment system.
Other duties as assigned.
Qualifications and/or Experience:
College degree or equivalent preferred
Min. 3-5 years work experience in health care agencies.
Certification in case management or equivalent experience
Certification in CPR
Valid California Drivers License and proof of current automobile liability insurance.
Computer proficiency
Excellent oral and written communication skills
Excellent organizational skills
Ability to compile data and write summary reports.
Experience utilizing an Electronic Medical Records (EMR) system is highly desirable
