2. About the Opportunity:
The Hennepin County Case Manager role involves providing client-centered case management services to individuals receiving CADI/BI and DD waiver services. The position advocates for clients and connects them to community resources, contributing to their independence and quality of life. This role is critical in supporting adults and children with developmental disabilities and mental health challenges, ensuring their needs are addressed through effective case management.
3. Responsibilities:
• Provide case management services to individuals on CADI/BI and DD waivers
• Manage caseloads averaging 35 to 43 clients
• Meet with clients two to three times per year
• Serve a client mix of adults and children depending on waiver type
• Advocate for clients and connect them to appropriate community resources
• Document client interactions and case notes accurately
4. Requirements:
• Bachelor’s Degree in social work, special education, psychology, nursing, human services, or related fields
• Social work licensure required if degree is in social work
• One year of experience with developmental disability populations or completion of Relias I/DD course
• Experience or strong interest in working with adults with serious and persistent mental illness
• Commitment to diversity, multiculturalism, and culturally responsive practice
• Valid Minnesota Driver’s License with acceptable record
• Ability to pass DHS background study
5. Pay Range and Compensation Package:
• Starting pay ranges from $50,000 to $52,000 per year depending on qualifications
• $1,500 hiring bonus for external candidates
