Role Overview: Supports patient navigation through post-acute care, enhancing outcomes and satisfaction. Develops patient-centered transition plans using evidence-based guidelines and protocols to ensure quality and efficiency. Represents the organization in strategic relationships with health systems, hospitals, physicians, and stakeholders.
Responsibilities: Assists with care transitions, integrates clinical and preventative metrics, and promotes positive patient outcomes. Builds relationships with healthcare providers and community resources.
Qualifications & Skills: Graduate of an approved nursing, therapy, or social work program; licensed in the practicing state; two years’ field experience; familiarity with healthcare laws and regulations; basic technology skills. Preferred: RN or PT, three years’ experience, healthcare sales background.
Requirements: Valid driver’s license, dependable transportation, automobile insurance, and ability to drive safely in all weather conditions.
