Role Description The Foster Care Case Manager is a full-time, on-site role located in Mount Washington, KY. This role is responsible for managing a caseload of children and foster families, including assessing needs, developing individualized service plans, and coordinating therapeutic and support services. The Case Manager conducts regular home visits, monitors safety and well-being, documents progress, and maintains accurate records in compliance with state and agency standards. This position collaborates closely with foster parents, biological families when appropriate, service providers, and state agencies to support placement stability and positive outcomes. The Case Manager also participates in team meetings, court-related activities, and ongoing training to ensure quality, trauma-informed care.

Qualifications

  • Ability to conduct thorough assessments, develop care plans, and coordinate services within a child welfare or behavioral health setting.
  • Strong skills in relationship-building, collaboration, and communication with children, families, foster parents, and multidisciplinary teams.
  • Competence in maintaining detailed documentation, case records, and compliance with regulatory and accreditation standards.
  • Skills in crisis intervention, conflict resolution, and trauma-informed support for children and caregivers.
  • Proficiency in using computer systems and case management software, along with solid organizational and time-management abilities.
  • Bachelor’s degree in Social Work, Psychology, Counseling, or a related human services field
  • Experience in foster care, child welfare, or behavioral health services and familiarity with Kentucky regulations is beneficial.
  • Valid driver’s license, reliable transportation for home visits, and ability to meet agency background check and safety requirements.
