Job Summary
The Eligibility & Support Services (ESS) Specialist serves as the first point of contact for individuals seeking Support Services at AID Atlanta. This position coordinates initial member engagement, schedules and completes intake appointments, determines member eligibility for Ryan White Part A and EHE programs, assigns referrals to Service providers, and ensures accurate documentation in the e2AIDAtlanta electronic medical record (EMR) system. The ESS Specialist supports seamless access to services while maintaining compliance with agency policies, funding requirements, and confidentiality standards.
Duties & Responsibilities
Member Intake & Eligibility
- Schedule and complete initial intake appointments for members requesting Support Services.
- Conduct initial intake assessments and complete the Initial Individual Service Plan
- (ISP), identifying barriers to care, determining service needs, and coordinating referrals to appropriate Support Services.
- Determine eligibility for Ryan White Part A and EHE programs and departmental services in accordance with program and agency guidelines.
- Collect, verify, and upload required eligibility documentation into the e2AIDAtlanta EMR.
- Ensure all required intake and eligibility documentation is complete and accurate.
- Enrollment into EMR-related programs as appropriate.
- Notify members of any outstanding documentation needed to complete eligibility.
Referral Coordination
- Complete and maintain the departmental Referral Tracking Log following Departmental referral SOP.
- Assign referrals to the appropriate Service provider based on agency procedures.
- Communicate referral updates with management as needed.
Administrative Support
- Maintain accurate electronic records and documentation.
- Cross-train to serve a secondary Front Desk Registrar.
- Provide exceptional customer service to members, visitors, and community partners.
- Compliance & Professional Development
- Maintain confidentiality in accordance with HIPAA, Ryan White, EHE, and agency policies.
- Stay current on Ryan White Part A, EHE requirements and HIV education.
- Attend required meetings, trainings, and professional development activities.
- Agency Engagement
- Participate in and volunteer for intra- and interdepartmental events, including AIDS Walk Atlanta, member-centered events, community outreach activities, and other agency initiatives.
Qualifications
EDUCATION & WORK EXPERIENCE
- Minimum Education: High School Diploma or GED.
- One (1) year of experience in customer service, healthcare, human services, or support services.
- Preferred: Experience working in healthcare, HIV services, or a community-based organization.
- Highly skilled in navigating and documenting within Electronic Medical Record (EMR) systems.
- Experience with Ryan White-funded programs is preferred but not required.
- Must pass criminal background check.
