Our HOPE Program offers early intervention services to children, ensuring they receive the best start in life.

Job Description: We are seeking a dedicated Early Intervention Service Coordinator to join our team. The Service Coordinator will manage services for children enrolled in the HOPE Program, ensuring they receive comprehensive support. This role involves processing referrals, maintaining records, coordinating services, and working closely with families to develop Individualized Family Service Plans (IFSPs).

Key Responsibilities:

Service Coordination: Serve as a service coordinator to assigned children and their families.

Referral Processing: Process new referrals and maintain child records.

Documentation: Document case management/service coordination provided, and any other information pertinent to the child’s well-being and case.

Service Coordination: Coordinate services for children enrolled in the HOPE Program.

Review Meetings: Conduct 6-month and annual reviews, as well as transition meetings.

IFSP Development: Complete Individualized Family Service Plans (IFSPs) with families.

Evaluation and Assessment: Evaluate, assess, and maintain program participant records.

Progress Review: Review monthly progress reports and attendance.

Eligibility Evaluation: Evaluate pending and annual referrals to determine eligibility.

Additional Duties: Perform other related duties as assigned.

Qualifications:

Education: Bachelor’s degree which meets AEIS Professional Standards required.

Knowledge: Working knowledge of typical child development.

Skills: Proficiency in Microsoft Word and Excel, excellent written and verbal communication skills.

Organization: Ability to prioritize effectively and maintain efficient organization.

Bilingual (Fluent Spanish and English)

Physical Requirements:

Must be able to physically maneuver with small children in positions that include sitting, standing, kneeling, crouching, and crawling at times.

Occasionally required to sit.

Specific vision abilities required include close vision, distance vision, color vision, and depth perception.
