Breaking Ground operates nearly 4,000 units of housing across New York City, along with housing in upstate New York and Connecticut. Supportive housing – affordable housing paired with services designed to help people maintain their homes for the long-term – is widely recognized as a proven and cost-effective solution to chronic homelessness.

But we didn’t stop at providing housing. Breaking Ground’s programs and services help people experiencing street homelessness – especially those who have been on the streets the longest – to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.

Connect to Care is a suite of programs that bring Breaking Ground’s approach to street outreach into privately managed spaces and discrete geographic areas. Through contracts with corporate partners and consortiums, Connect to Care works to discover people experiencing homelessness who have fallen through cracks in the system and get them connected to vital services and housing opportunities. Our approach treats people with dignity while helping them get on the path to stability.

POSITION SUMMARY:Reporting to the Senior Director, the Director, Programs, Connect to Care, is responsible for a portfolio of contracts funding 24/7 outreach and housing placement work in multiple areas including Midtown, Lower Manhattan, and Brooklyn with the potential for expansion to other boroughs. The Director maintains consistent and ongoingcommunicationwith various funders, creates high-level reports that detail the program’s performance metrics and effectively communicates that data to external facing partners.

With the ultimate goal of moving people off the street and into transitional and permanent housing, the Program Director works collaboratively with the Manhattan Outreach Consortium (MOC), and Breaking Ground’s Street to Home programs, other outreach providers, city agencies, and a broad array of community stakeholders to help reduce street homelessness. The Director oversees outreach activities and program operations while supporting Harm Reduction and Housing First strategies as well as other best-practice clinical interventions.

The Director providesleadershipto the program by informing decision making, implementing policies and procedures, and facilitatingeffective communication. The Director establishes and maintains positive andcollaborativeinterdepartmental relationships.

The Director works primarily during business hours Monday-Friday, with off hours availability in case of emergency or for special projects/initiatives. The Director provides oversight of 24/7 operations andflexibilityin work schedule as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Oversee a team of approximately 45-55 staff across multiple contracts and locations in a 24/7 program and is responsible for outreach and housing placement activities including special operations and joint initiatives with city agencies and other stakeholders, targeted initiatives to expedite housing placement and other interventions, follow-up with clients placed in stabilization beds and other transitional housing, etc.
