POSITION
PURPOSE:Work with the Social Worker Department at CHI Bergan Mercy. Use a strength-based approach that utilizes conversation andempowermentmethods to help resolve a household’s housing crisis or quickly connect them to existing emergency or crisis housing services, by working alongside the participant in an empowering manner. This conversation is to explore with the participant a creative, flexible, safe, and cost-effective solution to quickly resolve the participant’s housing crisis—even if just temporarily—with limited or no financial support.
This service is provided regardless of perceived needs and barriers.
ESSENTIAL JOB FUNCTIONS:
Coordinate and conduct intake and housing assessments for all participants.
Access each participant to ensure that they qualify for Togethers services and funding.
Gather documents from participants (IDs, social security cards, income verification, etc.) if they have such documents to file.
Advocate on participants’ behalf to property managers.
Gather documents from property managers (Deposit forms, past due rent forms, ledgers, leases, and W9s).
Contact, make pledges, and obtain bills from utility companies on behalf of participants.
Request checks from accounting to stabilize housing crisis. Process and prepare checks to be delivered or mailed to property managers or utility companies.
Develop an individualized goal plan for each participant.
Process and access referrals from social workers at CHI Bergan Mercy.
Provide support services in a non-judgmental manner.
Document referrals and services in an Excel spreadsheet shared with CHI Bergan Mercy staff.
Document services and funds in Clarity under the program for CHI Crisis Engagement services only.
Provide information, referrals, andadvocacyto assist participants in accessing services and resources.
Maintain participant-related data tracking systems, including case notes and complete HMIS entries.
Track services and input check numbers if needed on Front Door Excel spreadsheet.
Prepare case-related reports including outcomes, successes, and challenges.
Respond to community requests.
Attend meetings, as requested.
Communicate effectively with local community and external agencies; successfully foster relationships, which enable needed resources to be accessed.
Complete internal and external referrals, as needed, for supportive services and work collaboratively as a team to advance participants’ goals.
Demonstrate knowledge of available community resources.
Respectparticipants’ dignity and confidentiality.
Adhere to the facility dress code and appear professional.
Maintain records in a professional, confidential, and orderly manner.
KNOWLEDGE AND SKILLS:
Ability to engage and build strong working alliances with program participants.
Ability to work in both ateam environmentand independently with minimal supervision.
Ability to respond to stressful situations in a flexible and professional manner.
Strong organizational skills,attention to detail.
Exceptional oral and writtencommunication skills.
Computer skills, including Microsoft Office and database management.
