POSITION TITLE: Coordinated Entry Specialist
REPORTS TO: Coordinated Entry Program Manager
SALARY: $27.30 – $27.82 Bilingual
BENEFITS: Full benefits package includingMedical, Dental, Vision, Life Insurance, and Employee Assistance Program (EAP), plus Paid Time Off (PTO) encompassing holidays, vacation and sick leave (approximately 270+ hours annually), and 401k employer contributions after 1 year of employment.
HOURS: Full time 35 hours per week; typically, Monday-Thursday 9:00AM – 5:00PM, Fridays 9:00-2:00PM. Occasional nights/weekends.
LOCATION: This position is based out of our West Olympia location and includes working from community partner locations as needed.
About Us: Family Support Center of South Sound is a nonprofit 501c3 multi-service agency located in Olympia, Washington, founded in 1992 to provide families and survivors of domestic violence, sexual assault, stalking, and human trafficking with coordinated supportive services in one centralized location with the mission, “Working Together to Strengthen All Families and Survivors”. Family Support Center seeks to build strong, healthy, safe, and hopeful families through collaborative programs and services, with the goal to reduce the impacts of family and child poverty, homelessness, and family violence in the South Sound community.
Position Summary:The Coordinated Entry Specialist is responsible for supporting households in mitigating their current housing crisis. This may include problem solving conversations to identify a creative housing solution that diverts the household from the homeless response system, and connecting to internal and external financial resources. For households that cannot be diverted from the homeless response system, the staff member will complete Coordinated Entry intake, assessment, and prioritization for shelter and housing services. This position will support families with children, survivors of domestic violence, and single adults. Family Support Center is the lead Coordinated Entry provider for Thurston County, and this position must remain up to date with current policies and procedures, availability of community resources, and ensuring households are routed to the correct internal or external program as quickly as possible.
Preferred Skills and Experience:
Ø Education: Associates degree in social services or related field. May substitute education for comparable work experience.
Ø Experience: A minimum of 1 year of experience working directly with households experiencing homelessness
Ø Skills: Ability to write and speak effectively with all audiences
Ø Technology: Computer literacy; Proficiency in the use of Microsoft office programs.
Ø Bilingual, preferably in English and Spanish
Ø Experience with HMIS database and other electronic client tracking systems such as Apricot and Ragic
Ø Transportation: Insured personal vehicle and be willing to travel locally
Ø Local work experience; familiarity with Thurston County social service resources
Ø Experience working with diverse populations, knowledge of harm reduction, low barrier services, coordinated entry requirements
Licenses and Other Requirements:
Ø Washington State Driver’s License
Ø Fully insurable driving record
Ø Ability to pass a comprehensive background check
Ø Current DV/SA advocacy certification or willingness to obtain certification upon employment
Primary Responsibilities:
Ø Assess and identify options and strategies to divert households from the homeless crisis response system, including identifying potential diversion resources, opportunities to increase income, or overall housing stability
Ø Complete intake and applicable assessments for all households who cannot be diverted from the larger homeless response system, including families, survivors, and singles
Ø Coordinate and participate in outreach activities to support increased service to underserved populations residing throughout Thurston County
Ø As applicable, complete verifications of homelessness, income, and children’s status with clients upon intake
Ø Support in offering resources and referrals to households identified by CE Placement or Data staff, documenting attempts and outcome of offers
Ø Provide CE Intakes via phone as well as in person on a walk-in basis, remaining flexible and responsive to the needs of those seeking services. Average of 4 intakes per day
Ø Support in answering the coordinated entry line during business hours; answer questions of callers, provide applicable resources, and if necessary, schedule for intake/assessment appointments with next available staff person
Ø Co-locate onsite at community partners to complete coordinated entry intakes and assessments as needed. Locations may include Veteran’s Hub, Sergio’s Place, Peer Olympia, Maple Court, and North Thurston Family & Youth Resource Center
Ø Work from a housing first, harm reduction, and low barrier perspective, ensuring households are screened in and not out
Ø Participate in necessary trainings and continuing education opportunities as directed by supervisor
Ø Support in managing of the master list(s) to ensure households remain active, housing statuses are updated and correct, and moving households off of the list as they secure housing
Ø Represent Family Support Center at community meetings such as Coordinated Entry Committee, Name by Name, Built for Zero, Anchor Community Imitative, etc.
Ø Partner with community outreach teams to increase outreach to households experiencing unsheltered homelessness and facilitate greater access to CE Intake and Assessment for this population
Ø Work with CE team to identify and intentionally direct outreach to households disproportionately represented in the homeless system.
Ø When time permits, begin developing housing barriers assessment and individualized goal plan with families and survivors
Ø Directly connect households with access to resources to meet their immediate needs including: shelter, food, clothing, transportation resources, diapers, hygiene supplies etc.
Ø Assess households for various program eligibilities to ensure they are connected to as many services as possible, such as chronic homelessness for PSH and Foundational Community Supports services, shelter, rapid re-housing, HEN, etc.
Ø Ensure all data entry is complete following each Coordinated Entry intake, including thorough case notes so next staff member is able to easily pick up where CE Specialist left off, reducing trauma on household
Ø Maintain accurate and thorough electronic files. Retain documentation and case notes of services provided Ie: copies of referral forms, HMIS release, completed housing or benefits applications, vulnerability assessment, provisions provided etc.
Ø Enter data into the HMIS, Apricot, and Ragic systems as applicable. Ensure proper data quality and timeliness
Ø Support with processing intake/assessment packets completed by community providers on behalf of coordinated entry; follow up directly with client and/or provider when information is missing or incorrect
Ø Support with Built for Zero efforts to address chronic adult homelessness such as meeting participation, implementing tests of change, etc.
Ø Participate in agency’s on-call rotation to provide backup support to Pear Blossom Place staff in evenings; approximately 3-4 weeks per year
Ø Participate in weekly 1:1 supervision, regular Coordinated Entry team meetings, and all staff meetings as scheduled
