Role Description The Community Health Worker is a full-time, on-site role based in the Clark-Champaign county. This position supports community members by connecting them with health and social services, providing education on wellness and prevention, and helping individuals navigate healthcare systems. Day-to-day responsibilities include conducting outreach in neighborhoods and community spaces, meeting with women and gorls of reprodictive age to link to Managed Care Organizations and to assess needs, and offering guidance on resources such as primary care, behavioral health, housing, and food assistance. The role also includes maintaining accurate records of interactions, collaborating with local agencies and service providers, and contributing to community health initiatives and events. The Community Health Worker will work closely with colleagues and partners to promote health equity and improve outcomes for diverse populations.

Qualifications

  • Strong interpersonal, communication, and active listening skills to build trust and rapport with community members.
  • Ability to conduct outreach, provide health education, and explain complex information in clear, accessible language.
  • Organizational and documentation skills to track referrals, services, and outcomes accurately and confidentially.
  • Familiarity with local health and social service systems, community resources, and basic public health concepts.
  • Demonstrated cultural humility and ability to work respectfully with individuals from diverse backgrounds.
  • Capability to work independently in the field as well as collaboratively with multidisciplinary teams.
  • High school diploma or equivalent required; post-secondary education or training in public health, social work, or a related field preferred.
  • Experience in community outreach, case management, or human services is beneficial.
  • Valid driver’s license and reliable transportation to travel within the Clark and Champaign county.
  • Must be a CHW or willing to enrolll in paid training
