Responsibilities
- Conduct outreach initiatives to connect with culturally diverse communities, including underserved populations, to promote health awareness and program participation.
- Provide health education on topics such as nutrition, Medicaid, Medicare, chronic disease, and other illnesses.
- Assist clients with navigating healthcare systems, including understanding public health laws and policies, accessing medical services, and utilizing EMR/EHR systems for documentation.
- Support program development efforts by collecting data, maintaining accurate records, and assisting in community engagement activities.
- Utilize motivational interviewing and effective client communication skills to encourage healthy behaviors and lifestyle changes.
- Organize and deliver public speaking engagements or workshops on public health topics to educate community members.
- Collaborate with healthcare professionals, social workers, and community organizations to coordinate services tailored to individual needs.
Qualifications
- Experience working with individuals with disabilities or in social service environments.
- Knowledge of nursing practices, medical terminology, and basic life support certification.
- Proficiency in Microsoft Office applications for data collection and report writing.
- Bilingual skills are highly desirable to effectively serve diverse populations.
- Demonstrated experience in community outreach, engagement, or program planning within public health settings.
- Excellent written and verbal communication skills for client interaction and documentation purposes.
- Ability to work independently as well as part of a multidisciplinary team in fast-paced environments.
- Familiarity with addiction counseling, nutrition education, or public speaking is a plus.
