We have a full-time fully grant-funded position available for a Community Health Coordinator that will be based in West Des Moines, Iowa. This role is a vital part of our first-of-its-kind Mobile Clinic breaking barriers to healthcare access and making a tangible impact in communities across Iowa. UCS Healthcare is a CARF-accredited, non-profit health care organization based in West Des Moines, Iowa with additional offices in Knoxville and Ankeny and medication units across the state offering medication assisted treatment (MAT) for opiate/alcohol use disorder. Reports To: Chief Advancement Officer

Job Summary: The Community Health Coordinator will assist the Grant and Outreach Team to provide end-to-end project management while remaining aligned with the organization’s strategy, commitments, and goals. They will also facilitate patient access to grant funding and coordinate our Mobile Medication Assisted Treatment (MAT) Unit outreach initiatives. This role involves project planning, workflow administration, and stakeholder engagement to enhance community access to treatment services. The Community Health Coordinator will also identify and establish partnerships to support project execution and outcomes.

As part of UCS Healthcare, team members will follow all laws and regulations, including those at the Federal, State, and Local level. They will also adhere to all UCS policies and procedures, including patient confidentiality, HIPAA/ 42CFR part 2, and all CARF requirements. All team members will operate in alignment with UCS Healthcare’s core values, work effectively in a team environment, provide or support patient-centered services, and strive to assist UCS Healthcare in delivering the highest level of care.

Essential Functions (80%):

Work with Mobile MAT Unit staff to provide outreach around Iowa
Complete Patient Intakes and Substance Use Assessments
Assist Patients with grant funding
Provide Screening, Brief Intervention, and Referral to Treatment (SBIRT) Assessments
Manage outreach initiative logistics, including supply inventory and preparation tasks
Communicate with the Grant and Outreach team and ensure all members are coordinated with delegated tasks.
Deliver projects on time, ensuring quality standards are met.
Ensure resource availability and allocation.
Use appropriate verification techniques to manage changes in project scope, schedule, and costs.
Measure project performance using appropriate systems, tools, and techniques.
Report and escalate to management as needed.
Manage relationships with all stakeholders involved in the project.
Perform risk management to minimize project risks.
Create and maintain comprehensive project documentation.

Other Functions (20%):

As needed, assist other departments in developing new partnerships, projects, and engagement reports.

Minimum Required Education, Skills & Experience:

Temporary Certified Alcohol and Drug Counselor (tCADC) or Certified Alcohol and Drug Counselor (CADC) with Iowa Board of Certification.
Knowledgeable about substance use disorders, harm reduction and medication-assisted treatment (MAT).
Ability to drive or willingness to obtain training to operate an RV for outreach purposes.
Demonstrated ability to work collaboratively in a team environment while adhering to agency policies and procedures.
Proficient in Microsoft Office Suite (PowerPoint, Excel, Word, etc.)
Strong organizational skills.

Preferred Education, Skills & Experience:

Full CADC
Presumptive Medicaid training
SBIRT Training
Experience in leadership roles, community engagement, or establishing partnerships within healthcare or social services sectors.
A four-year degree.
Experience in Project management.
Experience with Outreach.

Physical Requirements: Travel is required (estimated 70%, no overnights). A valid driver’s license is required (an Iowa driver’s license is required within 30 days of hire) and liability insurance or an equivalent means of personal transportation and coverage as travel within the state of Iowa may be necessary. Communicating with others to exchange information. Occasional lifting and carrying equipment, boxes, medical supplies, or other materials.
