Primary Work Location:

The Kelly Cullen Community is located in the Tenderloin neighborhood of San Francisco and is home to 174 individuals with extremely low incomes, including those that experienced homelessness and are currently living with health challenges. On-site social services are run by the Tenderloin Neighborhood Development Corporation, clinical services are provided by Cardea Health.

Job Summary:

The CNA/HHA provides essential care and support to patients as part of a comprehensive care team including Registered Nurses, Licensed Vocational Nurses, and community providers. Responsibilities include assisting patients with activities of daily living such as bathing, dressing, and grooming, monitoring vital signs, feeding patients, and helping with mobility. CNAs/HHAs also provide emotional support to patients and report any changes in patient condition to the nursing staff. CAN/HHAs play a crucial role in ensuring patients’ comfort, safety, and well-being at the Kelly Cullen Community.

Job Duties & Responsibilities:

The CNA/HHA will assist clinical staff and KCC residents in the coordination of medical and behavioral health care. Duties include:

Conduct patient/personal care in accordance with resident plan of care
Support resident relationship with primary care and other professional disciplines
Assist nursing team in communication with residents including arranging and facilitating appointments with nursing staff
Ensure adherence to infection control standards on site among staff and residents
In collaboration with the housing agency, ensure daily wellness of residents. Participate in immunization programs at the KCC
Assist in the execution of population health and safety interventions on site
Answer calls to nursing station and respond independently to when appropriate
Assist nursing staff with light clerical duties including filing and making copies
Ensure that clinical documentation is current, timely, and accurate
Maintain confidentiality in accordance with the Health Insurance Portability and Accountability Act (HIPAA)
Recognize and respond to medical emergencies on site, follow up with MD communication or 911 activation
Practice harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect
Other duties and responsibilities as required

Minimum Required Skills & Qualifications:

High School Diploma or equivalent
Must be authorized to live and work in the U.S.
Must be 18 years of age or older
Must have personal, reliable transportation to and from work sites and shifts

Preferred Qualifications:

Experience with homeless or incarcerated populations is highly desirable
Two years of experience in patient care
Self-directed and able to work with minimal supervision
Detailed oriented and highly organized
Compliance with all company policies
Must provide proof of COVID vaccination or willing to be vaccinated
Prior experience with homeless or incarcerated populations is highly desirable
Proficiency in Microsoft Outlook, Word and Excel is required

Physical Requirements:

Frequent sitting or stationary work
Frequently lift and/or move up to 20 pounds.
Occasionally lift and/or move up to 50 pounds

Work Environment:

Employees have direct contact with clients/patients while administering care.
This position will require traveling to other office locations or off-site meetings as required by the job.
Employees may be occasionally exposed to toxic or caustic chemical

Compensation and Benefits:

Competitive Rate of Pay (Hourly Pay is $28.56 – $29.58)
