Overview
The Case Manager at the Stapleton Corps will work to develop partnerships with other social service agencies, churches, and other not-for-profits across Staten Island while strategically utilizing those relationships to cultivate comprehensive, individualistic plans of recovery and healing for the families being served through the Social Ministries of The Stapleton Corps.
Responsibilities
Cultivate partnerships with other social service providers, churches, and other not-for-profits across Staten Island that offer services that could assist clients.
Perform casework duties, which include meeting with clients seeking agency services, conducting intake and assessments, brief counseling, and information and referral.
Develop comprehensive service plans and make periodic reports to the supervisor on the progress of each.
Carry a caseload and perform record-keeping functions such as detailed progress notes and census statistics.
Maintain all records with strict confidentiality.
Refer residents to agencies and programs as necessary and establish a rapport and professional relationship with these organizations.
Attend staff meetings as requested by the Corps.
Attend training sessions and workshops around the NYC area as requested by the Corps.
Create a process for measuring the impact of personalized plans created for clients and measure each case at least two times per year.
Prepare workshops and educational opportunities for clients, focusing on relevant topics that support their recovery and healing.
Work closely with other staff at the Stapleton Corps.
Review and determine approval for various forms of support (e.g., clothing vouchers, rent assistance) based upon budget.
Oversee the Pantry Aide, including scheduling, payroll input, and disciplinary actions.
Work Expectations
Consistently report to work on time and be ready to begin work at the start of the shift.
Provide proper notification when late.
Demonstrate reliability when on-call and be available to work overtime when necessary.
Recognize priorities in the organization of workflow.
Able to perform duties independently, with minimal need for direct supervision.
Maintain the confidentiality of clients, shelter/program, agency, and/or The Salvation Army.
Report to work well-groomed and in compliance with dress code.
Interact in a positive and helpful manner with clients, visitors, other staff, volunteers, and co-workers.
Commit to building a supportive work environment and maintain a positive attitude at the workplace.
Project a good image when dealing with the public and clients.
Be willing to make an extra effort to help build a quality case management program.
Qualifications
Bachelors Degree in Human Services or a related field.
Demonstrated experience and knowledge working with underserved populations on Staten Island or the surrounding boroughs.
MSW candidate preferred, but not required.
