Case Manager – Keeping Families Together Program
Youth and Caregiver Services Division
Position Located in Bernalillo County
Pay Rate: $21-24 Hourly/ $43,680-$49,920 Annually
Requirements:
-
Bachelor’s degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology, Family Studies/Services, or a related field.

At least 2 years of relevant experience, which may include community-based services, social work, case management, behavioral health, health care, or related work.

Bilingual Spanish/English a plus.
Responsibilities:

Conduct home visits using the KFT Tiered Case Management Model.

Complete trauma-informed comprehensive family assessments to identify strengths, supports, needs, and safety concerns.

Help families obtain and maintain safe and stable housing.

Connect families with employment, education, behavioral and physical health care, financial assistance, legal advocacy, and other community resources.

Conduct formal and ongoing safety assessments and develop safety plans when appropriate.

Assist families with budgeting, housing needs, transportation, and access to approved emergency resources.
