Salary: $21.75 – $24.50

Summary:

The HUBS Case Manager will provide case management, outreach, and support services for older adults, especially those living within the Baltimore Citys central and Bel Air Edison neighborhoods. The case manager is responsible for conducting client assessments, assisting clients with accessing grant and loan funding for home repairs & modifications, and providing wraparound social service referrals. The HUBS Case Manager will be responsible for maintaining accurate records on all ongoing cases, completing monthly, quarterly and annual reporting, and attending meetings and outreach events in support of the program.

Essential Duties and Responsibilities:

Identify, contact, and assist a minimum of 75 low-income older adults per year with issues such as home repair, home modification, or weatherization services

Connect older adults to other housing services, which include handyman services, tax credits, foreclosure prevention, and tax sale prevention through the completion of city, state, and nonprofit applications

Assist older adults in applying for resources or benefits that make their homes safer and increase their value

Provide quarterly and annual program, and financial data reports to management as required;

Maintain records and report activities to funding sources

Assist with budget preparation and monitoring for programs

Collaborate with community, city, state, federal, and other social service agencies to support program goals

Create cohesive programming to achieve the projects selected by program participants and aligns with neighborhood goals as appropriate

Advocate with agencies or persons to help clients receive appropriate benefits or services

Identify and recruit volunteers

Expand existing programs and identify additional resources

Performs related duties or responsibilities, as assigned or requested.

Qualifications:

Required:

Bachelors degree in social work, human services, or gerontology, or equivalent combination of education and work experience

Practical interpersonal skills with experience working with diverse populations;

Must be highly organized and self-motivated with practical time management skills

Must have dependable, insured transportation and a valid Maryland Drivers license and acceptable driving record; willingness to drive throughout Baltimore City required

Desired:

Experience conducting needs assessments and initiating support services

Sensitivity in working with older adults preferred

Ability to understand and follow complex written and oral instructions, rules, and procedures

Ability to work independently and to be a vital and contributing part of a team

Ability to effectively handle associated administrative responsibilities; including dealing with funder requirements, maintaining records, and meeting reporting requirements

Ability and commitment to handle privileged information professionally and confidentially

Strong written and oral communication skills

Excellent problem-solving skills: Strong computer skills

Ability to motivate and connect with others successfully

Physical Requirements:

This position normally requires the physical demands of standing, walking, bending, lifting or performing other work requiring medium exertion, talking, and hearing on a regular basis to perform the job requirement. These physical demands are required up to 85% of the time.
