About the Role:
Case Managers provide an integrative approach to coordinating the care of their clients. Working with a defined caseload, case managers partner with the client to devise a treatment plan to meet their individual goals and objectives toward improving their lives. Case Managers are the backbone of the organization, living out our mission of “Improving the lives of the people we serve” on a day-to-day basis.
Responsibilities Include:
– Developing treatment plans
– Providing information and referral
-Advocacyand home visits
– Follow-up services as defined by individualized goals
– Engaging in employment support, child development, family support, HIVeducation, and more.
Requirements:
– An F-endorsement is required as many positions provide transportation as necessary.
– Driver’s License and access to a personal vehicle for local travel is required for virtually all positions.
Salary:
Starting salary ranges from $15.93-$20.05/hour depending on position,education, and relevant experience.
