Residential Services Case Manager

Position Overview:

The primary role of the Residential Services Case Manager is to broker community resources, conduct individual case management, assess client needs, advocate, and facilitate group activities for formerly homeless, disabled adults. The goal is to help clients maintain housing and prevent homelessness.

Essential Job Functions:

Orient new residents and assess their strengths and needs.

Develop and update service plans.

Link residents to services and support housing stability.

Assist residents in moving towards more independent housing.

Coordinate with property management and clinical staff.

Organize social and educational activities.

Complete verification paperwork and provide crisis intervention.

Transport residents and cover for other case managers as needed.

Qualifications:

Strong interpersonal and communication skills, emotionally intelligent.

Knowledge of homelessness issues and sensitivity to underserved populations.

Ability to negotiate, organize, and make sound decisions while maintaining confidentiality.

Knowledge of local resources, mental health, and substance abuse treatments.

Ability to advocate effectively and develop measurable treatment goals.

Valid driver’s license and ability to pass background checks, including driving a 15-passenger van.

Education & Experience:

Bachelor’s degree in social services or related field preferred.

Experience working with disenfranchised populations.

Valid driver’s license.

Compensation:

$20.64 – $25.14 per hour, based on experience.
