Role Description The Case Manager is a full-time, hybrid role based in St Paul, MN, with some work-from-home flexibility. This role involves providing person-centered support to individuals with disabilities, including assessing needs, developing individualized service plans, and coordinating resources such as housing, employment, benefits, and care services. The Case Manager will maintain regular contact with participants, monitor progress toward goals, and adjust plans as needed to ensure effective support and independent living outcomes. Daily responsibilities include documentation, collaboration with community partners and service providers, advocacy on behalf of participants, and participation in team meetings and trainings. The Case Manager will uphold MCIL’s values of choice, dignity, and inclusion in all interactions.
Qualifications
- Ability to conduct needs assessments, develop individualized service plans, and coordinate services and resources to support independent living.
- Strong communication and interpersonal skills, including active listening, clear written documentation, and collaborative work with diverse stakeholders.
- Knowledge of disability rights, community-based services, and person-centered approaches to supporting individuals with disabilities.
- Organizational and time-management skills to handle multiple cases, maintain accurate records, and meet deadlines and reporting requirements.
- Proficiency with basic office software and case management or data tracking systems; ability to work effectively in a hybrid (in-office and remote) environment.
- Experience in social services, human services, counseling, or a related field; bachelor’s degree or equivalent combination of education and experience preferred.
- Commitment to equity, inclusion, and non-discriminatory practices, with the ability to work respectfully with people of varied backgrounds, cultures, and identities.
- Knowledge of local community resources in the St Paul, MN area; multilingual skills are a plus.
