Job Description:
Harbor Care Foundation (HCF) is a non-profit organization that provides short-term residential care for homeless individuals who are recovering from an illness or injury whose condition would be worsened by living on the streets. We provide a safe haven for residents who are physically independent to recover and connect them to vital services and permanent supportive housing.

Summary
The primary role of the Case Manager is to provide support, guidance, and the necessary tools for our residents to live a healthy and independent lifestyle—physically, emotionally, and spiritually. In addition to traditional case management responsibilities, this role includes life coaching to help residents overcome personal challenges, build resilience, set meaningful goals, and develop the confidence and skills needed for long-term stability and reintegration into the community.

This is a full-time, regular exempt position reporting to the Floor Manager located in Mission Hills, CA. Compensation is commensurate with skills and experience. This position is eligible for benefits according to applicable guidelines.

Essential Duties & Responsibilities

  • Provide assessments and case management for all residents.
  • Participate in/facilitate case management meetings with program staff to collaborate on resident transition plans.
  • Oversee assigned residents’ stay from move-in to exit and transition.
  • Work with residents to create individualized case plans and life coaching goals related to housing, employment, financial management, education, life skills, therapy, spiritual care, and personal growth.
  • Act as a Life Coach by motivating residents, helping them overcome challenges, build resilience, and identify purpose-driven goals.
  • Offer motivational support, helping residents build self-awareness and confidence, and guiding them through barriers to achieving independence.
  • Assist residents with referrals to other service providers.
  • Facilitate disciplinary and exit meetings with residents.
  • Maintain accurate and timely documentation, including case notes, referrals, outcomes tracking, and reports.
  • Communicate with Floor Managers and Care Unit staff about safety concerns, program compliance, and other resident issues.
  • Attend collaborative meetings with external providers and represent HCF in community partnerships and coalitions.
  • Assist and coordinate transportation for various appointments and meetings for residents, which will include driving the residents.
  • And other duties related to the operation of the organization when necessary.

Supervisory Responsibilities
May supervise volunteers or interns on occasion.

Other Duties & Responsibilities

  • Participate in the daily routine of facility/residential life.
  • Participate in special events and facility tours.
  • Participate in trainings and staff development activities.
  • Attend other meetings as assigned.

· Completes additional tasks/duties as required to meet the needs of the organization, not to impede primary responsibilities.

Qualifications

  • Bachelor’s degree in psychology, sociology, Social Work or Human Services preferred.
  • Graduate of accredited program preferred.
  • 2 years of experience in a healthcare or social services setting preferred.
  • Experience in case management, social work, counseling, or life coaching.
  • Knowledge of HMIS, Clarity, or similar social service databases.
  • Understanding of common medical conditions.
  • Strong interpersonal, analytical, and time management skills.
  • Ability to handle multiple projects and work independently.
  • Bilingual (English/Spanish) preferred.
  • Valid California driver’s license and insurable under HCF.
  • Willing to work flexible hours Monday through Friday, including some evenings.

Physical Demands
Office-based work with regular use of phone and computer. Includes transporting clients to appointments.

Work Environment
Office setting within a respite care facility serving medically vulnerable individuals. Must be comfortable working in a high-need environment with potential exposure to illness and unpredictable behaviors.

Job Type: Full-time

Pay: $23.00 – $25.00 per hour
