We’re looking for a compassionate, organized Case Manager to join our team and help connect clients with essential services like ID recovery, hygiene items, transportation, and referrals.
What You’ll Do:
Assess client needs and determine eligibility
Provide direct support and resource navigation
Maintain accurate case documentation
Build relationships with community partners
Support our mission with empathy and professionalism
What We’re Looking For:
High school diploma (degree in human services a plus)
Experience in social services or nonprofits preferred
Strong communication and organization skills
Lived experience is valued and welcomed
